You must apply separately to your jurisdiction's Radiation Protection / Radiation Health department.
Each jurisdiction has its own form to complete to apply for a licence that is well documented on their websites.
What to include in your application:
- Certificate of Training - received when you successfully completed the course. The certificate has a QR code that the regulator will use to verify your completion
- Competency Checklist (if applicable) - signed by your supervisor to attest to your ability to apply the training in practice
- Face-to-face exam results (if required by your jurisdiction)

The licence process and any fees are separate to your training.
